Community Interactions Inc.

Community Interactions Inc. (CI), founded in 1971, provides person-centered services to individuals with autism, intellectual disabilities, and behavioral health challenges. In 1973, CI opned its first home in Delaware County, Pennsylvania. Today, Community Interactions serves over 250 adults in a variety of community living arrangements and inclusionary day programs in the States of Delaware and Pennsylvania.

Community Transit

Community Transit is a private, non-profit organization that provides reliable, affordable, transportation to residents of Delaware County through their mission "to increase the mobility and indepence of senior citizens, and persons with disabilities." Community Transit's service is available to everyone in Delaware County for any trip purpose. The Senior Shared Ride Program is a reduced fare transportation program available to riders age 65 and older who are willing to share their trip with other passengers. Community Transit is also the county-designated administrator for the Medical Assistance Transportation Program in Delaware County. ACCESS card holders may call 610-490-3975 for more information. Under the PwD program, transportation is available for people with disabilities, age 18-64 who need transportation from an area not served by existing transportation services. Passengers must become registered with Community Transit by calling their office at 610-490-3977 or by visiting their website at www.ctdelco.org . Once registered, passengers can begin scheduling trips by calling 610-490-3960. Trip requests are accepted up to 12 noon, one day in advance, as availability permits. All trips are reserved on a first-come, first-served basis and rides are available as capacity and ride-sharing allow.

Freedome Valley Disability Center

Freedome Valley is a Center for Independent Living that has been serving people of all ages with disabilities in Chester, Delaware, and Montgomery Counties for nearly twenty years. The services provided include: Information and Referral, Advocacy, Independent Living Skills Training, and Peer Support. We help access transportation, home modification programs, and social opportunities, among other things. FVDC is staffed by individuals with disabilities, adding a unique peer perspective to service provision.

Mercy LIFE

Mercy LIFE is a nationally recognized Program of All-Inclusive Care for the Elderly (PACE) that offers older adults a total solution for long-term care. Our program provides medical and social services, at home and at the Mercy LIFE Day Centers, helping older adults maintain their independence. Mercy LIFE's compassionate team of healthcare professionals, specializing in geriatric care, work with individual's and their family's to evaluate and develop a customeized care plan to meet each individual's personal needs and goals.

To enroll in Mercy LIFE

  1. You must be 55 or older
  2. Need the level of care that requires nursing facility type services
  3. Live in Delaware County or one of the following Philadelphia County zip codes: 19106, 19107, 19122,19123, 19125, 19133, 19134, 19137, 19140, 19145, 19146, 19147, or 19148.
  4. Need medical care and supportive services and are able to live in the community safely with the services provided through Mercy LIFE.
