   Aging Waiver
This Waiver Program offers older adults an alternative to institutional care by providing long-term care services to qualified Pennsylvanians age 60 and older living in their homes and/or communities.  
 

     Attedant Care
This Waiver Program provides services to eligible persons with physical disabilities in order to prevent institutionalization and allow them to remain as independent as possible.  
               

         Act 150
Individuals who are not eligible for a HCBS Waiver Program may be able to get non-medical support services in their home through Act 150

 

Payment to Family Members

Family members (other than a spouse or parent/step-parent of minor child) may receive payment to provide Personal Assistance Services. This can be helpful in situations where it is the preference of the individual receiving Waiver services and/or because it is the preference of their family.

 

 

DISABILITIES CARE
 

There are three Waiver programs that fall under the Community Services Program for People with Disabilities (CSPPPD).
 

 

     CommCare Waiver

This Waiver Program provides services in the community to eligible persons with a traumatic brain injury in order to prevent institutionalization and allow them to remain as independent as possible.
 

         

OBRA Waiver

This Waiver Program provides services to persons with developmental disabilities so that they can live in the community and remain as independent as possible (this includes relocating or diverting individuals from a nursing home to a community setting).   
                    

 


        Independence Waiver
This Waiver Program provides services to eligible persons with physical disabilities in order to prevent institutionalization and allow them to remain as independent as possible.

 

COMMUNITY INTEGRATION

 

 

Supported Employment

Waiver participants may be eligible for additional supports from specialized vocational providers. These supports include individual job development and job placement assistance, on-the-job training, training in related skills essential to obtaining and retaining employment, and transportation assistance. 
 

Community Integration

Community Integration Services are services designed to assist individuals in acquiring, retaining, and improving self-help, socialization and adaptive skills necessary to reside in the community. Community integration services and related training enable the recipient to plan for and engage in
meaningful adult activities including domestic and leisure activities.

 

 

 

Home and Community Based Services (HCBS)


Home and Community Based Services are also known as Waiver Funded Services or Waiver Programs. The name waiver comes from the fact that the federal government "waives" medical assistance rules for institutional care in order for Pennsylvania to use the funds for HCBS. HCBS provides for supports and services beyond those covered by the Medical Assistance (also referred to as Medicaid) program that enable a person to remain in a community setting rather than being admitted to a Long Term Care Facility. Currently the Department of Public Welfare administers 12 HCBS programs. Each HCBS program has its own eligibility requirements and services.

 

 

Our Agency (HomePlus Care Org)

 

Our agency works with the Medicaid Home Community Based Services for the Frail Elderly (HCBS/FE), the Physically Disabled (HCBS-PD), Developmentally Disabled, and the Traumatic Brain Injury Waiver (HCBS-TBI). These are programs that are authorized through Department of Human Services and Department on Aging of five counties: Philadelphia, Mongomery, Delarware, Bucks and Chester. We are Medicaid certified through the Pennsylvania Department of Health and Human Services and contract with most private insurances. With locations in both Upper Darby and South Philadelphia, we service agency-directed clients within 50 miles of Philadelphia, Pennsylvania and FMS clients state-wide.
